Our business office staff is highly trained to assist you in making your financial arrangements and to help you in dealing with your insurance company to achieve the maximum insurance benefits. A variety of financial arrangements will be discussed with you during your initial office visit, including payment in full at time of visit, several financial plans and third party coverage (insurance).
Treatment at Michiana OMFS may be covered by major dental or medical insurance plans depending upon your particular needs and circumstances. We ask that all patients provide our receptionist with pertinent medical and dental insurance information during their first visit.
In an effort to keep costs down while maintaining a high level of professional care, we have established the following payment policy for the use of our patients. Fees are based on the complexity of the procedure and are relative to the level of care extended, and may be paid as follows:
- Payment for your initial visit is always expected at the time of the service. Michiana OMFS accepts cash, cashier checks and all major credit cards.
- Financing is now available through Care Credit for patients who qualify. Care Credit offers up to 12 months at no interest (same as cash options), and also extended payment plans for flexible financing.
- Accounts outstanding more than 60 days from treatment may accrue interest and/or rebilling fees. The rebilling fee will recur as long as a balance is due. Credit inquiries are done on a routine basis.
As a service to you, we will process all medical and dental claims, including billing medical insurance for all surgical procedures and dental insurance for treatment not covered by medical insurance. Our staff will assist you with your insurance issues in any way possible, keeping in mind that we must always honor the contract terms between you and your insurance provider.
