Why do I need an evaluation?
During the consultation, your medical history will be reviewed to determine whether there are any health problems that may affect your surgical treatment and your X-rays will be reviewed to view the position of the teeth and determine if there are any special concerns. This initial visit enables you to be fully informed about what is involved in your surgery. Anesthesia options and the results you can expect after treatment can be discussed. We also provide the estimated cost of your treatment and payment options during this meeting.
What do I need to bring to the initial consultation?
-Your surgical referral slip
-Any X-rays (if applicable)
-A list of medications you are presently taking
-If you have medical or dental insurance, bring your cards and the necessary completed forms
